When using both Mac and iOS, with The Old Reader service, items marked as read on iOS will still appear as unread on Mac, and during the next sync on iOS, those read items will also turn back to unread.
Update:
The actual problem turns out to be that the items marked as read on iOS didn't sync successfully to The Old Reader.
However, items marked as read on iOS won't revert to unread upon refreshing. But once sync with the Mac app, and then open the iOS app, the read status will revert to unread.

When you mark them as read on iOS, and then go to Old Reader itself, are those articles read or unread?
Awesome! I never considered that it might be a problem with the iOS app. Indeed, as you pointed out, the items marked as read on iOS didn't sync successfully to The Old Reader.
