Teddi Mellencamp Arroyave is sharing a disappointing update on her melanoma journey.

On Monday, The Real Housewives of Beverly Hills alum, 42, posted a video on her Instagram Story explaining that the immunotherapy treatment didn’t work for the melanoma on her back.

“I just had a big ol’ cry,” she said while letting out a big sigh. “I just got the call from my doctor that the immunotherapy did not work, it was not successful.”

“So I have two different options,” Mellencamp explained. “I can either do a big skin graft of my stomach or my arm but it’s a very long recovery or I can go in and get another wide excision surgery on December 26. And they’ll have a plastic surgeon come in too because it’s such a big area and then just keep getting my checks every three months.”

“So that’s where we are. Get your skin checked folks,” she added.

The health update comes shortly after Mellencamp clapped back one follower who criticized her appearance as she’s been going through treatment.

On Sunday, she posted a photo dressed up with husband Edwin Arroyave as the couple attended an 80s-themed holiday party. However, one Instagram user commented that her “face looks sickly.”

“Your face is too thin! You look like you've been starving yourself!” the user wrote before Mellencamp replied, “It was an 80’s themed party and I have been on immunotherapy treatment for my melanomas. Makes me feel sick. But I hope it made you feel better to comment the above. So kind.”

The commenter later apologized for the hurtful message, sending well wishes for her health.

On Sept. 13, Mellencamp announced on Instagram that she'd been diagnosed with her thirteenth melanoma in the past year. 

Since then, Mellencamp — who was first diagnosed with stage 2 melanoma in October 2022 — has been open about starting an immunotherapy cream as a part of her treatment plan. 

"I was given the option of doing a surgery and having my entire right shoulder's skin removed and replaced with a skin graft or using this immunotherapy cream. My margins weren’t clear after my last surgery so this is what makes most sense. I chose the cream to start," she wrote on an Instagram post in October, alongside a video clip showing her 11-year-old daughter, Slate, applying the cream to her mom's shoulder.

Mellencamp told PEOPLE in December 2022 that her experience with the disease has inspired her to use her voice and encourage others to be vigilant when new spots appear on their skin and keep up with their doctor's appointments.

"Skin checks are essential," she said at the time. "We've seen how quickly it can turn from one melanoma to 12. So when your doctor says, 'See you in three months,' it's three months."
