    August is National Wellness Month, so we’re taking this opportunity to highlight the importance of regular, routine appointments with a primary care doctor 🏥.   Regular wellness checks with a primary care physician can prevent more challenging health conditions in the future by assessing lifestyle, evaluating health risks, screening for health conditions to catch potential risks early.    That’s why one of the goals of Care Coordination, from Help at Home is to successfully connect home care to health care – we connect our clients to their PCP to ensure they understand the importance of wellness visits.     To date, we’ve connected 70% of hard-to-reach, dual-eligible clients in the program to their PCPs. #HelpatHome #HomeCare #NationalWellnessMonth

    Ohio-based caregiver Naomi, has been working with the same client for 26 years.  Naomi started caring for her 45-year old developmentally disabled client when she was a teenager. As you can imagine they have a very special bond. “The longer you are with someone, the more they become like family,” said Naomi.   We’re very proud of Naomi and caregiver-client relationships like theirs across our nearly 60K caregiver network in the communities we serve.
