Health and Wellness is a wave, and integrated care will be (and already is!) the defining characteristic of most winners. Many DTC healthcare companies are putting money into Meta/Google, and getting patients out. Something similar is possible at a Doctor's office, with a significantly lower CAC. Put in resources, get patients out. I think this is a compelling distribution channel for a lot of wellness companies--Pharmacies, Gyms, Nutritionists, Therapists, Longevity etc. Because if you're a healthcare company, chances are the people who you can impact the most (and who may be the most motivated to use your product!) are those at the Doctor's office. Yes it will be non-trivial to set up distribution through clinics... but those nuances are just a barrier to entry for your competitors. The rise of multi-clinic operators makes this extra powerful right now.

Today was a big win for consumer health, with Flo Health Inc.'s valuation at $1 billion. We're hearing talk about the market becoming frothy again, but I’d argue—without seeing the detailed financials—that this valuation could be entirely warranted. Flo has built an impressive business, with 5 million paid subscribers and 70 million monthly active users. At $60 per year for users to access the product, this business has hundreds of millions in revenue and plenty of room to grow. It's increasingly clear to me that innovators focused on healthcare have been wildly underestimating consumer business models for years, believing that patients would only opt for solutions covered by their health plans and prescribed or recommended by their doctors. That thesis needs updating, particularly given high deductible plans and soaring co-pays. Innovators building solutions that users genuinely love will win - even in healthcare. These businesses also have the potential to move into B2B channels, which is far easier to do when you have an engaged, loyal user base. I expect we’ll see many more consumer health businesses “pop” in the years to come, further proving that consumers are ready and willing to pay for such solutions. We're watching closely at Cherish Kudos to the team at Flo for this impressive milestone. Let the wave begin! Pioneer Fund company Recall.ai *grew their revenue 10x* in the past year, has 300 enterprise customers, and just raised a $10M Series A led by Ridge Ventures. David Gu and Amanda Zhu dropped out of college at age 19 to start the company. They've grown it exponentially with a tiny team. They make it easy to build meeting bots that are cross-platform. They're aggregating all these different APIs into one unified API, the way Twilio did with cell phones. At first it "sounds" easy to build a meeting bot -- just integrate with the Zoom API. But in fact there's a huge long tail of things you need to build, security and privacy pitfalls, automations, transcription complexities, integrations, the calendar invites where someone is using Google Meet or Teams, staying up-to-date with the libraries, etc etc. Recall takes care of all of that. It's WAY easier to build on top of Recall.ai and focus on the features that make you unique. Congratulations to the Recall team!

Day #4 of observations from attending Home Care 100 this month: There is a cambrian explosion of "AI scribe for OASIS docs in <15 min" startups currently underway in Home Health. A new one of these is launching every few months. I'm certain we'll see 2x AI scribe vendors at the next HC100 in January. And so, I listened for the overall positioning of these products by the vendor and buyer at Home Care 100. It sounded like this: HH CEO: “I need a solution to lower the caregiver burden of clinical documentation” HH Vendor: “I offer AI to reduce the burden of clinical documentation” This may sound like a "checkmate" match between what the CEO is saying, and what the vendor is offering. But it isn't. The CEO actually has a business model problem. They cannot afford to sufficiently pay for the time it takes for caregivers to complete clinical documentation. And so, they are stuck looking for solutions that minimize the cost of personal time on documentation that caregivers have to absorb. Solutions that offer AI and the promise of driving down this cost down for the caregiver are the start, but not the endgame. If I were not building Arya, and wanted to build a standout documentation product, I would build an "AI-powered clinical documentation agency", not an AI scribe. Here's what this AI-powered documentation agency would sound like: 1. We eliminate 90% of all clinical documentation at your company. We do it at 1/10th the cost of a traditional outsourced documentation shop 2. We do this by a human-in-the-loop AI solution, that allows our human documenters to work 10x faster than DIY 3. Under the hood, we integrate with *all* the best AI out there (incl. ChatGPT) in a SOC2+ HIPAA guaranteed environment. We decide the best AI+human workflow to use for each documentation on the fly. Regardless, you always pay a flat rate, and we absorb the risk or AI vs human effort 4. Your caregivers approve each human-in-the-loop documentation. We don't publish anything to the EHR till it's approved 5. We guarantee a <1% error/ change rate on our documentation from caregivers, or we offer some money back I did some back of the napkin math to see what the economics of an such an agency might look like. With the current math, the caregiver is still absorbing upwards of $10 worth of time per OASIS documentation. If an AI-powered agency drove this cost down to the single digits per doc, might HH agencies be able to work this into their financial model and take this off the caregiver's plate entirely? ADI and SVI did not match up well. How about SDI and SVI? A few days ago, I compared the Area Deprivation Index (ADI), a widely used health equity index by the Centers for Medicare & Medicaid Services, against the Social Vulnerability Index, developed by the Centers for Disease Control and Prevention (CDC) [2]. In the post, I pointed out that these two indices correlate to some degree, but there are many areas where SVI views are vulnerable, but ADI views fine - mismatches between the indices. For those who want to look at my previous post, click the reference [2] link. Today, I want to introduce another health equity index—the Social Deprivation Index (SDI) developed by the American Academy of Family Physicians [3]. Like many other indices, SDI is based on U.S. Census Bureau data and provides deprivation scores at the census tract, Primary Care Service Area, ZIP, and county levels. So, let's look at the relationship between SVI and SDI. They are both provided at the census tract level, so no transformation is needed. The chart below shows the relationship between SVI (y-axis) and SDI (x-axis). SVI and SDI have slightly different focuses, so their scores would not perfectly match up. However, in most cases, low SDI means low SVI and high SDI means high SVI. In other words, they are highly correlated. At this point, please take a moment to look at my previous post: [2]. Among these three health equity indices, SVI and SDI are reasonably similar, while ADI behaves a little bit differently from the other two. Why do we see such differences? There could be multiple reasons. However, the root cause is behind the data ingredients used to build these indices - that's my view. One particular variable in ADI is not present in SVI and SDI. I’m increasingly convinced that AI value is going to accrue to companies that deliver services in healthcare. 3 specific places: 1) micro private equity that acquires companies and then injects AI into their operations. Traditional success in acquiring healthcare businesses has been around negotiating better rates, controlling referrals, etc. But with AI maybe you can reduce the operating expense, improve margins, and just hold the business while it generates cash. This would probably work particularly well in areas where you need several full time staff to do administrative work like clinical trial sites, durable medical equipment suppliers, legacy third-party administrators, etc. Less so in places that have strict staffing ratio requirements. 2) outsourced departments that’re powered by AI. For example, hospitals are now trying to completely outsource their billing departments. A company can come in and be willing to take over all the headcount/back-office services themselves and do it for much less thanks to AI. For example, instead of 10 medical billers the hospital hires as full time employees, a company uses 1 medical biller + AI and charges 50% of the cost. 3) unbundling a package of services and completely automating part of it. A made up example, legal firms where you can choose between a lawyer reading a contract for $X or an AI they trained reading a contract for 20% of the cost. Or InPharmD, which is essentially a clinical pharmacist + AI which they use to create different products (ask a complex question about drugs, or for a hospital committee to generate reports on what drugs they should keep on hand).

InVision has experienced the biggest fall from glory in Silicon Valley's recent history. $350M raised with a peak valuation of $2B. And now they are shutting down. Not acquihired or "acquired by private equity" or strategic tuck-in. Just a hard stop. It's so easy to s**t on them. They got out-innovated, they were not product driven etc. etc. But, the truth is most startups that raised monster rounds in ZIRP days could face this deadend. You raised too much money: 1/ So you are too expensive to acquire 2/ So you raised your burn too high to finance a big GTM team 2/ So you got complacent on product 3/ So you got lazy on shipping There will be a lot more inVisions in the next 24 months as startups run out of capital. It's sad. The employees & founders at these companies gave it their all, but raising too much money sealed their fate. For the next gen of startup founders, heed this advice: raise little, keep your burn low, ship fast!

I 💯agree with Oz Merchant on this topic. Some folks advise founders to pitch very selectively and 'pick your pitch' - you get one shot, choose it wisely? I'm 180 degrees opposite of that - be a promiscuous pitcher! - ABP - ALWAYS BE PITCHING. Because your pitch gets better with every no, take that feedback, learn, iterate. Right now it's taking 50-100 no's for Triangle co's to get funding, it's still tough out there, so get through the no's fast to get to a yes. Check out the event - also go see others pitch, it all helps you get better - pitching is like PMF - lots of iteration, tons of grinding to get through to the other side.

AI Therapist Services Surge and the Drama: Concerns At Geridoc, we've been diving deep into the world of Large Language Models (LLMs) and exploring their potential in revolutionizing medical diagnostics. Our team has been experimenting with a variety of models, including GPT-3.5, GPT-4, and PMC-LLaMA, to support doctors in their daily endeavors. From aiding in accurate diagnoses to suggesting treatment paths, and even crafting crystal-clear emails to enhance patient-doctor communication, we're on it. As a really small company, the journey ahead is exhilarating, sometimes scary. Yet, there's a sick twist on the horizon. Come 2024, we're noticing a surge in startups and indie hacker services that seem to be making a quick buck off the decline in mental health, by launching AI therapist services. Yes, you heard that right. 🤯 Drawing from my experience working alongside mental health professionals and general physicians, particularly in the wake of COVID, here's my two cents: Post-COVID, Southern Europe has seen a spike in social phobia and depression diagnoses, driven by factors like fear, financial struggles, grief, and social isolation. Enter Big LLMs. They've become a mainstream marvel, offering invaluable assistance to therapists. As Blyler and Seligman (2023) put it, “ChatGPT enhances the work of the therapist and coach by doing the ‘busy-work’ of generating a narrative identity and outlining a panoply of useful and evidence-based techniques for the human coach/therapist to capitalize upon.” However, let's be clear: LLMs aren't ready to replace therapists. There's a whole can of worms regarding ethics, liability, and patient data privacy that needs addressing. In my view, AI therapist services are essentially capitalizing on social isolation and phobia, potentially hindering timely access to crucial, specialized mental healthcare. With no regulation in sight, the future of such services is murky at best. Particularly concerning are those developed by indie hackers, who may not prioritize safety and guidelines. Tools that delay access to specialized care shouldn't even be on the table. Mental suffering is not a commodity. Let's not treat it as such. #AI #Therapists #AITherapists #MentalHealth

Do you know how many hyper-growth public software companies are growing by over 40% this year? According to the latest June Guggenheim report… Zero. Not a single public SaaS company is growing over 40%. Do you know how many are growing their next twelve months (NTM) revenue over 30%? Also, zero! With that context, I was strategizing with my team on the most impactful ways companies can change their growth trajectory, and one thing we landed on solving immediately was time to integrate. If a company growing at 30% can pull in all net new integrations and thus revenue by just 1 month over the course of the year, it can increase its growth rate by almost 5%. For simplicity, assuming a $100M annual revenue, a 10% discount rate, and a 5-year valuation period, using a DCF, the company’s present value would not be 5% more valuable, but closer to 20% more valuable for all shareholders. The power of compounded growth rates. Game changer, and with just the smallest adjustments. That’s why smart companies are investing heavily in new customer onboarding and no-code, low-code technologies, like lightweight agents, hosted or embedded UIs, and more.
