The West Coast-based health system is deploying Innovaccer’s healthcare AI platform and population health management tools — starting with its Washington market and potentially expanding to new markets in the future. "San Francisco-based Innovaccer was founded in 2014. The healthcare technology company specializes in data-driven solutions that help providers enhance patient care and operation efficiency. "Its healthcare AI platform helps simplify patient care by bringing together data from various disparate sources. By combining all this information, the platform provides a complete and detailed view of each patient’s health, explained Innovaccer CEO Abhinav Shashank. “This helps doctors, nurses and other healthcare professionals get a clearer understanding of each patient’s situation and deliver better care. "We have standard noncompete provisions within our contracts for our affiliated providers. We feel these are needed to help protect both parties as the organization has invested in the startup and growth of the clinic and the providers have invested their time in building patient panels and closely held relationships with their patients." from Novant Health, in its response to RFP to privatize New Hanover Regional Medical Center, March 2020. This is the same Novant Health that is pushing back on the FTC's suit to block its acquisition of more hospitals in North Carolina. Novant now owns New Hanover Regional Medical Center (more on this later). How pervasive are physician non-compete clauses that limit physician mobility, entrepreneurship, and potentially patient access? If North Carolina is any example, they are very pervasive--at least among the mega-health systems. In 2020, 5 mega-systems bid to acquire New Hanover Regional Medical Center, a profitable, highly attractive county hospital in Wilmington, NC. All 5 bidders--Novant, Atrium, HCA, Sentara, and Duke had to answer the question in their proposals, and all 5 acknowledged they require physician noncompetes in their system-owned practices. Because NHRMC was a public hospital, the proposals had to be disclosed publicly. Non-competes are usually non-disclosed. Here is what HCA said in its proposal: HCA typically includes non-compete language in its physician contracts. The terms and  conditions vary based on a number of factors including but not limited to size of the group, specialty, state laws and regulations, and local market dynamics.  Here is what Atrium said: Atrium Health believes convenants against competition are more market competitive than cost-sharing provisions. To remain competitive, Atrium prefers the use of covenants but is willing to consider alternatives. . . " In 2018, a group of Charlotte physicians that became Tryon Medical Partners sued Atrium to get out of an unfair noncompete. They are now a successful independent medical group, but it hasn't been easy. The founder in part credits shared savings programs, noting, "one thing that has helped to level the playing field significantly is shared savings programs which created a way for primary care to stay in business." It sure seems like physician non-competes are a tool in the arsenal of anti-competitive practices observed as hospital consolidation occurs in and across markets. I’ve been given advice that the fastest way to monetize HER Heard without charging the end user (woman) is trackers and selling data. Instead, I’d like to build something more trustworthy. I don’t want to be in a headline like this. If you are not paying, then you are the product (you/your data are being sold). There are newer ways to deindentify data or even to aggregate things women search for that can be of interest to various stakeholders. In theory that could be okay to sell, but in a post-Roe world with laws encouraging bounty hunting, I am vetting the choices ahead. I am all for moving fast, without breaking things.

At its inaugural, Margaret Chan, then D.G WHO, called " Universal Health Coverage is the single most important concept that public health has to offer." Yet a decade later there has been no progress on financial protection and almost none on service coverage and outcomes. And these problems precede and continue after the Covid Pandemic. In this conversation we look beyond implementation issues to more basic questions about UHC as strategy and as a discourse.

A while back I re-shared an article about the FFS and PMPM rates that would be needed to cover the costs of operating a community health worker (CHW) program by the Waymark research team. This article by the California Health Care Foundation describes how California's CHW program was developed and what they went through, including barriers (like merits of certification, CHW reimbursement rates). It also paints more context on the growing role (and need) of the CHW— a trained specialist with lived experience who helps patients in underserved communities navigate the health system and connect to social supports. The article highlights the key component best: "For generations, CHW/P/Rs have filled critical gaps in California’s health landscape and acted as liaisons between marginalized communities and mainstream systems. As trusted community representatives, they share cultural and linguistic backgrounds with their clients, many of whom are immigrants, people of color, and people with low incomes. And they provide accessible information and support in ways that resonate with their surrounding community." It would be pretty hard to replicate this connection and bridge with centralized care coordinators, discharge planners, customer service reps, etc., let alone AI, imho. As more closed loop referral systems are being implemented across city, county, state HIEs, and, systems, health plans, Medicaid agencies, etc., the increased effort on supporting whole-person care gives rise to more opportunity—and hopefully more funding pathways—for this kind of relevant, community-based work. Really great read by Robin Buller.
