Our 2023 Annual Report has launched! It’s no secret that Rhode Island’s primary care system is in crisis. Our industry is experiencing pressure at new levels in our post-pandemic world. At the Care Transformation Collaborative of RI, we have worked diligently to support primary care investment, bolster our workforce through new initiatives and programs, and deepen our community-centered work to improve care. In our 2023 Annual Report, we share powerful examples of the real work happening in RI through CTC-RI to address and respond to our industry’s needs. As we move through the year ahead, our efforts to transform our system continue. While we build, support, and advocate for advancements in primary care, we continue building a stronger, more resilient care system to support RI today and tomorrow.

Primary care is in crisis in Massachusetts, and we can all do more to support primary care teams. The Boston Globe’s recent editorial highlights the burdens facing primary care in the Bay State, and it is no secret that caring for complex patients is among the most challenging work for primary care teams. Senscio helps to relieve the burden on PCP by supporting complex Medicare patients. Patients referred to Senscio’s Ibis Health Program get coaching on self-management of their chronic conditions, increasing adherence to their PCP's care plan. Plus, remote monitoring increases PCP visibility into how that patient is doing between visits. https://lnkd.in/g3S8yK6F

The major shortage of nurses in hospitals across New York State is a major concern. We’ve made it our mission to uncover — and address — the reasons why. We’ve just launched our brand-new report with the Center for Health Workforce Studies on New York’s nursing shortage, exposing the major factors contributing to the shortage and the best strategies to address them. Through this research, we’ve confirmed one crucial fact: there is no single “silver bullet” to improving RN recruitment and retention. Most Maine hospitals struggle to be profitable; patients are forgoing care because they can’t afford it; and employers are just about tapped out on how many more dollars they can devote to employee benefits. It seems like the healthcare system isn’t working for anyone, so what can we possibly do? Nearly a year of collaboration with our Maine hospital systems to understand their delivery system finances has taught us that there’s no easy answer. The movement of dollars through the system is complicated (understatement of the year!), and no two hospitals systems are the same. Despite this understanding, though, we do believe that there are steps every hospital can take to improve the value of our healthcare system. At our upcoming event on June 6th, Steve Littleson, CEO of Central Maine Healthcare, will help us better understand the challenges facing our state’s hospitals, and our keynote speaker, Dr. Peter Pronovost, will outline the opportunity and strategy for making hospital care safer and more affordable for everyone.
