The Delusion of Scale in Healthcare Healthcare doesn’t scale - period. In fact, the only companies that have successfully scaled healthcare are the large insurance companies, and these do this through inter-company accounting transactions, medical loss ratios, rationing healthcare services from authoritative vertical integration. You don’t have to believe me, I only worked on more than several buy-side transactions for the two largest heath insurance companies. This is also why you had very few successful roll-ups of PE healthcare platforms - the majority failed. Here’s why: The cost of healthcare does not decrease with more volume. One provider can only serve X amount of patients, and then you must hire a new provider. Same with a location there is a capacity and then a new one must be added. True “fixed” costs are almost non-existent in healthcare, especially since most providers expect a compensation based on a percentage of cash collections (hopefully wRVUs but that’s for a different day). In healthcare it is just as important to understand your cost at the service/claim level as it is to understand your revenue cycle. Understand the cost and risk of your patient population and control expenses accordingly - that is a surer path to profitability than the delusion of “scale” ever will be.

The era of one-size-fits-all medicine is giving to precision medicine, which tailors treatments to individual patients based on their genetic makeup, lifestyle, and environmental factors. With advances in genomics, molecular profiling, and biomarker identification, precision medicine promises to target therapies with greater efficacy and fewer side effects. Investors are betting on big startups pioneering precision medicine solutions, recognizing the value they bring to improving patient outcomes and reducing health costs. When we started two years ago with a vision to empower small and medium-sized hospitals 🏥 , we weren't fully aware of the depth of impact we would create 🤔, as this segment was rarely discussed. While many digital health companies focused on patients as the core area, we understood that uplifting small and medium-sized hospitals was crucial. Without empowering them the experience can’t reach and ultimately it will affect patient care , any efforts for patient well-being would be incomplete and ineffective. Convincing others, was challenging 😥 , as competitors mainly focused on medicine delivery, telemedicine, elective surgery, and diagnostic . Introducing a new approach was met with resistance, and few were willing to listen to the insights we had and how to address them. 🥸 It's gratifying to see the industry and other bodies now echoing the same sentiments we envisioned😇. Even more so, it's encouraging to observe our competitors adopting our narrative. 💪This serves as significant validation for us. We are accelerating for 2024 with more power and focus 🧘‍♂️ At Mykare Health , we're not content to follow our competitors. In the Senior Care space, is focusing on their health & comfort enough? Not sufficient says Venkataraman Krishnan (my former colleague for 15+ years and good friend). He and the founding team at Prayojana Golden Years Services have built a disruptive model that looks at Senior Care in an entirely new way. Their clientele over the last 5+ years is very happy, proving that their disruptive hypothesis is on the money. After studying 1000+ disruptors, one of my biggest findings is that they don't solve problems, they solve what we call performance undersupply. Performance Undersupply can only be revealed by new unimagined expectations. The work being done by Venkat & team proves this finding about disruptors. I recently did an hour long interview with Venkat. I am going to be posting key segments from it over the next couple of weeks. Trending News | Global | M&A in Healthcare and Life Sciences: A Shrinking Margin for Error in Deals In 2023, deal volume declined across sectors, but deal value rose because of notable megadeals in pharma and medtech. The industry is sitting on high levels of cash—$171 billion across pharma companies. Also, top-line growth has a disproportionate impact on total shareholder returns (TSRs) in this industry. In pharma, for example, revenue growth has a seven times greater impact on TSR than margin growth, making inorganic growth especially attractive. We just released our H1 2024 Digital Health funding summary. https://lnkd.in/gAZY64tp H1 2024 Digital Health funding closed at $12.4 billion invested across 719 deals. - Q2 2024 is outperforming Q1 by 34% ($7.1 billion vs. $5.3 billion)! - Digital Health funding has shown sustained investor confidence. Funding in Digital Health in North America is up 14% to $9.5 billion. Investment in Europe is down 10% to $1.52 billion, while funding in APAC dropped by 7% to $1.17 billion. - Partnerships in Digital Health are flat in the first half at 1,730. NVIDIA has been the strongest corporate partner in H1 2024 - 96 M&As, 8 bankruptcies, 5 delistings from the stock market and 3 relistings from NASDAQ to OTC market. Really stimulating start to the day on a panel about ‘dark data’ (the untapped data in healthcare) at the Digital Healthcare Show. It was chaired by the brilliant Anushka Patchava MD, MBA. Some key takeaways - As we’re all consumers of healthcare do we have a responsibility to share our own data to ensure better outcomes for not only ourselves, but others ? - If we want to unlock the power of data then we need clinicians engaged, involved and empowered. - There’s huge promise in using data that we don’t currently collect, especially when it comes to logistics and operations. Walmart will close all its clinics, another blow to retail healthcare. Is this the end of Big Retail's attempt to transform healthcare delivery? Walmart cites the challenging reimbursement environment, escalating operating costs, and lack of profitability as the main drivers. They're also competing for the same scarce clinical resources as the rest of the country. I was hoping Walmart Health would succeed. Access to healthcare is limited in rural areas and the closing of hospitals has created a genuine concern for "health" deserts. Walmart would have created available healthcare within ten miles of the 60 million people living in underserved communities by offering accessible and affordable healthcare services through its primary care centers. Healthcare is hard. It's not the same as selling products like toothpaste or cereal. Perhaps Walmart didn't find enough #waste to cut? Perhaps there aren't enough incentives for healthcare clinicians to move to Rural America. Perhaps the unnecessarily complex and bloated reimbursement landscape was too overwhelming. John Kell's recent article in Fortune, "The best investment opportunities in AI for health care right now," provides incredible insights about the challenges of healthcare data and how it impacts the advancement of #ai. Three out of every four leading health care companies are experimenting with generative AI or attempting to scale it across their business. What are they creating and why? Venture capitalist Deena Shakir asks founders if they "are aiming to solve a technology problem or a clinical adoption problem". Solving existing problems will speed decision making and improve data quality. The Deloitte report:  “Navigating the Emergence of Generative AI in Health Care” shares some of the data challenge in health care.
