The successful outcomes of the Mayo Clinic model of care are well know in the healthcare ecosystem, yet it generally is outweighed by the hegemony of poorly conceived financial optimization criteria in the hospital industry. As Eric Bricker, MD suggests, the narrow financial criteria even undermine #salary-based #academic health centers when they become too large. Dr. Bricker does very briefly suggest one source of hope and growth in cases with excessive #uncertainty, #complexity or #ambiguity: "community physicians may refer you to an academic physician, but a lot of times the patient has to take into their own hands and do it themselves." The implicitly reflective and perhaps deliberative process outside of institutional reach must be made more #transparent so it can be addressed and aided by #science & #technology. We need a science for #innovation in #primarycare which is the only way to extend Mayo Clinic's model beyond institutional walls, to a wider population, and increasingly farther into #earlydetection and management of medical #risks. This must be a #scienceinthewild that starts with #reality as a way to be comprehensively grounded in reality. Sick care is not the story. It is an epilogue to the story of an individual's life.

Keratoconus is the most common corneal ectatic disorder worldwide. Currently, it accounts for nearly 30-40% of the corneal tertiary referrals, placing significant burden on the ophthalmic services. To address this issue, several innovative measures have been introduced, including the set up of virtual clinics and allied health professional (AHP)-led clinics. In our recently published article in the Eye journal, we highlight the effectiveness, safety and feasibility of AHP-led keratoconus service model (with minimal consultant support) in managing new and follow-up cases of suspected keratoconus. AHPs, including optometrists and nurse practitioners, were able to make the same correct decision as the corneal consultants in 92% of the cases!
