Check out Hello Sono's latest blogposts on #pocus and #medicolegal myths. In my conversations with many clinicians, in the #emergencydepartment and #primarycare, this subject often comes up. I get asked questions like: "Am I going to get sued?" "Am I putting myself and my team at risk for liability by doing POCUS?" "I'm not a radiologist, can I still make decisions?" So they either don't do it at all or they do it and not document it. I am not sure what's worse! Over and over again, POCUS has been shown to improve safety of procedures, increase diagnostic accuracy, and expedite time to diagnosis and treatment. And no, you don't need to be a radiologist to perform and interpret POCUS! Most providers know the evidence, but they still choose to avoid POCUS due to this fear of getting sued. In our blog post we review the current literature. The only lawsuits where POCUS was implicated had to do with failure to perform POCUS when it was available, not for misinterpreting the images. As long as you are following your subspecialty's and institutional guidelines and keeping up with your POCUS education, it is very unlikely for POCUS to become the cause for litigation. I have returned home after giving presentations at two fantastic professional conferences: Texas Society of Health-System Pharmacists (TSHP) Annual Seminar and Becker's Healthcare Annual Meeting. I met with old friends, caught up with fantastic colleagues, and met new leaders in pharmacy practice and healthcare. I am particularly energized following great attendance and feedback at my sessions. This innovative procedure used a single-spinal anesthesia similar to a C-section, allowing John to feel no pain and be discharged within 24 hours (compared to the typical 2-3 days). Here's why this is a big deal: Faster recovery: Discharged in less than a day, potentially making kidney transplants more accessible as out-patient procedures. Reduced risk: Spinal anesthesia avoids complications associated with general anesthesia, especially for high-risk patients. Many defense attorneys point to a signed and witnessed consent. They then declare that the patient agreed to proceed and thus the provider 𝘤𝘢𝘯𝘯𝘰𝘵 be blamed. 𝙏𝙝𝙞𝙨 𝙞𝙨 𝙛𝙖𝙡𝙨𝙚. Many plaintiff attorneys point to a signed and witnessed consent. They then point out that the complication was not listed and thus the provider 𝘪𝘴 𝘵𝘰 blame. 𝙏𝙝𝙞𝙨 𝙞𝙨 𝙛𝙖𝙡𝙨𝙚. It is simply impossible to consent for every possible outcome and variable that exists in medicine or that may arise peri-operatively or peri-procedurally. Agreeing to proceed implies that the more common risks have been discussed to some degree and that a patient is aware that they may occur. 𝗜𝗻 𝘁𝗵𝗲 𝗲𝗻𝗱: There is no guarantee of a good outcome in medicine. There is always a real or implied risk in healthcare interventions and this cannot and mustn’t be weaponized by either the plaintiff nor the defense side. More importantly: 𝙄𝙩 𝙙𝙤𝙚𝙨𝙣’𝙩 𝙬𝙤𝙧𝙠 𝙖𝙣𝙮𝙬𝙖𝙮. The Median Effective Dose of Intrathecal Hyperbaric Bupivacaine for Cesarean Section at Moderately High-Altitude https://lnkd.in/giK_p98h “`html Study Summary: Determining Optimal Bupivacaine Dose for Cesarean Section at High Altitude At moderately high altitudes, the need for local anesthesia during cesarean sections can be different due to altitude-related changes. This study aimed to find the right dose of bupivacaine for spinal anesthesia during cesarean sections at moderately high altitudes. Methods and Findings Thirty parturients at moderate altitude underwent cesarean section under spinal-epidural anesthesia. The study used a sequential method to determine the effective dose of bupivacaine. The median effective dose (ED50) of hyperbaric bupivacaine for spinal anesthesia during cesarean section was found to be 8.23 mg, exceeding the typical ED50 dose at low altitudes. Only 6 parturients experienced hypotension during the operation, and no adverse reactions such as nausea, vomiting, or shivering were observed. Conclusion and Implications The study concluded that the optimal dose of hyperbaric bupivacaine for spinal anesthesia during cesarean section at moderately high altitude is 8.23 mg. Further research is needed to determine the ED90 or ED95 dose of local anesthetics for cesarean section at moderately high altitudes to provide better guidance for clinical practice.
