With or without AI the burden of chronic disease on primary care providers will need to be reduced through preventive rehabilitation at home in between office visits. This requires new frailty prevention & self-management support telehealth expertise. All RPM and other telehealth services should include approaches that holistically strengthen patients by truly slowing age-related chronic disease and associated disability. Medications are not enough to reduce this burden on primary care and on patients and yet this is the main tool that has been tried with resultant poor outcomes and high costs relative to other countries. Primary care needs a new subdivision with expertise in multi-condition preventive rehabilitation. AI could help but only if it increases patient self-efficacy in how to they can get healthy. We have a moral obligation to deliver this for our patients.
