Can you define what sleep actually is? Highly enjoyed presenting on sleep, pain and behaviour at VOACON recently and getting stuck into the fundamentals of sleep medicine. If you’re interested, sleep (distinct from hibernation/coma/anaesthesia/seizure) is a rapidly reversible loss of consciousness, homeostatically controlled, and with a defined position. If you’re interested about learning more on sleep as a clinical team I run CPD sessions and I’m told they’re quite fun! Sleep Apnea Assessment: Do You Know Your Goals? (part 1) There is a growing profusion of tools for assessing sleep apnea. The latest issue of Montage (requires AASM membership to access) and a recent LinkedIn post by Anthony Warren (https://lnkd.in/guP-f8ae) outlines several. So many options – but which are viable for you as either a patient or sleep medicine practitioner? Answering requires knowing your goals, or "jobs to be done" for Clay Christensen fans. To me, there are three essential jobs. Keeping these in mind can explain why a sleep assessment solution is successful in one market, but fails in another. Job #1: Determine if Obstructive Sleep Apnea is a Problem You may have heard me previously say job one was answering “is there a problem?” The goal is actually narrower. If a patient or physician is considering sleep apnea assessment options, it is because symptoms already indicate there is a problem. So the first job is to see if OSA can be conclusively ruled in. Tools that are only able to “suggest a risk of OSA,” or “stopped breathing events” fail at this job. It would be great if the assessment could also rule out OSA, but I argue that currently, this is not a must. Current practice parameters require in-lab polysomnography to conclusively rule out OSA in a person presenting with OSA. The OSA positivity rate for patients presenting with symptoms is likely 80% or higher. This means most patients can get to diagnosis with a less expensive, easier-to-access, in-home sleep test. From the 29th of May to the 1st of June we took part in the "Mediterranean Meetings" congress, as a partner. This year's main topic was "Sleep & Metabolism", and the agenda focused on the physical impact non-restorative sleep can have on people.
