That’s the headline, and we don’t love it… but here are some of the lesser-mentioned positive proposals from CMS: -Advanced payments to MSSP ACOs with experience. That’s big for cash flow, which is a common pain point within the value-based care world! -Better incentives in MSSP for providers who serve rural and underserved communities. Let’s bring value-based care, and the wrap-around services it often includes, to patients in need! -Introduction of G2211 complexity code. Let’s reimburse more for managing longitudinal relationship complexity that isn’t captured in the work of standard E/M codes. -Creation of three new codes for advanced primary care management services that bundle elements of several existing care management and communication technology-based services. This could be the start of something big - truly paying for proactive care. Capacity (availability of appointments) is a major challenge in primary care. Keep reading below to learn more about how HealthHelper is expanding access to Medicare Annual Wellness Visits with South County Health! CHALLENGE: South County Medical Group (SCMG) recognized the value of providing Medicare Annual Wellness Visits (MAWVs) to their patients, but primary care providers had limited capacity to provide MAWVs to their large Medicare patient population. In response, SCMG developed a Nurse-Led MAWV Program to enhance their capacity. SCMG staff, however, were reticent to schedule appointments with this new program due to the time it required to educate patients on the benefits. SOLUTION: SCMG leveraged its partnership with HealthHelper to proactively identify patient candidates for this program, engage those patients, and schedule appointments with the Nurse-Led MAWV Program. HealthHelper leveraged AI-assisted, bi-directional text messaging and phone calls to engage and educate qualifying patients on the program. HealthHelper’s care coordinators, serving as an extension of the SCMG team, directly scheduled patients for their Nurse-Led MAWV appointments, ensuring strong utilization of the program, more accurate risk coding, and better care gap closure. #primarycare #medicare #populationhealth #AI #patientaccess #healthcarequality #annualwellnessvisits #healthcareIT
