When I interviewed at Social Finance 10 years ago, Jake Edwards Caitlin Reimers Brumme and Rebecca Leventhal asked me: what is your dream social impact bond? That was in the days when we thought we could just will cool ideas into existence. So for a decade I’ve been simmering this idea. Pretty much everyone agrees that much of what influences health happens outside of the *healthcare* system: it’s where you live - your moldy carpet and how close you live to the highway (or under the flight path of a small airport Cecilia D. Segal), but also how hard it is to access important stuff like food and doctors and childcare; it’s your job, education, the language you speak, how you get treated by others. Anyways, I’ve been participating with Stuart M Butler PhD and others on a Convergence Center for Policy Resolution project for the past 6 months. Among many other recommendations (link below) is to experiment more with outcomes-based funding. Healthcare leaders are pretty unclear about which things outside of their walls will keep people healthier. I think there's a real opportunity to let them test which ones work, and ask them to pay for those things when they do - to create an R&D fund that gets replenished when innovations actually achieve better outcomes. That will let us really use the giant, giant money of healthcare to drive better health.
