This week, the NTSB issued a new Safety Alert, Pipeline Safety Management Systems: Vital for the Safe Operation of Pipelines. The NTSB has considered pipeline operators’ adoption of safety management systems in several investigations, including four pipeline accident investigations occurring between 2010 and 2021: • Marshall, Michigan • San Bruno, California • Merrimack Valley, Massachusetts • Huntington, California As a result of these investigations, we found that pipeline safety would be enhanced if pipeline companies implemented safety management systems. This week, NTSB Transportation Safety Specialist Anthony L. presented on a public-use air accident case study to Federal Aviation Safety Officers at GSA's Interagency Committee for Aviation Policy (ICAP) Workshop hosted at NASA Glenn Research Center in Cleveland, OH. As public-use aircraft are exempt from conventional FAA oversight, Lam highlighted the importance for public-use organizations to operate with a robust safety culture, including the need for comprehensive SMS and risk management programs.
